  • New Patients: All all initial intake forms and consents will be sent to you via email but can also be found in your patient portal. All forms are required to be completed before seeing your provider for the initial visit.

  • Returning patients: Log into IntakeQ patient portal to view upcoming appointments, as well as make other changes/requests:

    • You can send your provider a secure message.

    • You can request medication refills.

    • Update demographic information.

    • Update billing information.

    • Pay your bills (BCBS and Tricare patients and self-pay only)

    • All other insurance will use Alma for billing and invoicing.

  • Telemedicine: You are able to see your provider virtually with smartphones, tablets, or computers. You will receive a link at the time appointment is set and you will also receive a link via email and text at the time of your visit when your provider logs in.
