  • New Patients: All forms needed for treatment with your provider will be shared with you through IntakeQ. These forms can be reviewed and electronically signed through the Documents tab in your IntakeQ patient portal. All forms are required to be completed before seeing your provider for the initial visit.

  • Returning patients: Log into IntakeQ patient portal to view upcoming appointments, as well as make other changes/requests:

    • You can send your provider a secure message.

    • You can request medication refills.

    • Update demographic information.

    • Update billing information.

    • Pay your bills (BCBS patients and self-pay only)

  • Telemedicine: You are able to see your provider virtually with smartphones, tablets, or computers. Instructions for joining telehealth visits can be found on your IntakeQ patient portal.

No mobile or email information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
